I, Samantha Salazar, Filipino, of legal age, single and a resident of 123 Hillside Street, Philippines, after having duly sworn to in accordance with law, hereby depose and state:
That I am the holder of an Automated Teller Machine card number XXXXXX under Account No. XXXXXX issued by ABC Bank;
That the credit balance of this account are 25,000 as of December 2010.
That I lost said ATM on January 8, 2010 and that I exerted all efforts to locate the same but I could not remember exactly where I placed it and it is considered lost for all purposes;
As such, I am executing this Affidavit of Loss to attest to the truth of the foregoing and to support my application for the issuance of a new Automated Teller Machine.
