1.    That I was born in Buenavista, Spain on November 20, 1977;

2.    That the name of my father is Francisco Saenz a citizen of Buenavista, Spain and the name of my mother is Josefina Saenz a citizen of Jordan, Spain;

3.    That I got my Spanish citizenship by birth;

4.    That I am a holder of Spanish Passport No. UU0306450 issued on December 19, 2007 in Spain;

5.    That I arrived in the Philippines on March 15, 2011 from Japan as a tourist.

6.     That the causes about the loss of my passport are as follow:

7.    I am executing this statement about the loss of my Spanish Passport to affirm the above mentioned facts that occurred and to request from the lawful authorities to issue my new passport.

That I lost said ATM on January 8, 2010 and that I exerted all efforts to locate the same but I could not remember exactly where I placed it and it is considered lost for all purposes;

As such, I am executing this Affidavit of Loss to attest to the truth of the foregoing and to support my application for the issuance of a new Automated Teller Machine.

That diligent effort to locate the Identification Card was useless;

That I am executing this affidavit to establish its loss and to support my request for replacement of the said Identification Card from ABC School.
