Dear Mr. Fox,
I wrote this letter to inform you that I am really disappointed on your behavior in the office. You always come in late and you are not following the policies of the company as well. We know that you have problems on your own but it must not affect your performance at work. We are also aware that this is the first time that these kinds of situations happened.
You are well aware that this kind of behavior has a corresponding punishment. You will be suspended for 3 consecutive days starting tomorrow. I hope that this punishment will make you realize your mistake. We are open to any concerns with regard to this matter. Thank you.
Respectfully,

Dear Mrs. Stone,
This is a written notification about your three-day suspension on March 25-28 2010. This disciplinary action is due to the physical fight that you had with Janice Henderson.
I have examined the circumstance carefully and I have interviewed Alex Miller. Alex said that you started the fight and you provoke Janice to fight against you. This misbehavior should receive enough attention. I am expecting that this kind of incidence will not happen again in the future. Any circumstances that are similar with this misbehavior will surely lead to your dismissal.
You can freely talk to the Human Resource Department about this matter.
Respectfully yours,
