It is disappointing to know that you have failed to report in your duty for one week without any further notice. This happened on March 15-19, 2010. You are then subject to unauthorized leave without any pay. As you know the company have a written policy that any three consecutive absences without any notice to your supervisor will be normally filed against you foe your dismissal. This will automatically means that you are having a voluntarily resignation because you abandoned your position.
This is appropriately subject against you. If there are any stories behind this absences that I should know please let me know. I will be waiting for three days for your response.

Dear Mr. Hall,
This letter is to inform you about my decision as your supervisor for your job dismissal because of your failure to submit to the following rules in our company.
- It is kept in your recode that you have received a written reprimand letter on August 5, 2009 because you have been arriving in your job late for three consecutive days in a week.
- On November 9, 9009 you have again receive a second written warning because the report you submitted for your job had been late and contain numerous error.
As agreed in your second letter, any repeated occurrence of the said violations will result to your dismissal. Recently on December 14, 2009 you didn’t report for your duty without further notice, and on December 15 and 16 you arrive on your duty one hour late.
