Dear Mr. Fox,
I wrote this letter to inform you that I am really disappointed on your behavior in the office. You always come in late and you are not following the policies of the company as well. We know that you have problems on your own but it must not affect your performance at work. We are also aware that this is the first time that these kinds of situations happened.
You are well aware that this kind of behavior has a corresponding punishment. You will be suspended for 3 consecutive days starting tomorrow. I hope that this punishment will make you realize your mistake. We are open to any concerns with regard to this matter. Thank you.
