Dear Mr. Walter,
On December 26, 2009 I purchased a humidifier model R16 from your ABC Company, in the amount of $70 to be charge to my Visa card.
Together with this letter are the copies of my receipt and my credit statements.
Upon checking the credit statement I see that your company has entered it into my account twice. You can see the both charges highlighted on the statement copy.
I would like you to take a further action with this duplicate charge in my account immediately.
