I, Samantha Salazar, Filipino, of legal age, single and a resident of 123 Hillside Street, Philippines, after having duly sworn to in accordance with law, hereby depose and state:

That I am the holder of an Automated Teller Machine card number XXXXXX under Account No. XXXXXX issued by ABC Bank;

That the credit balance of this account are 25,000 as of December 2010.

That I lost said ATM on January 8, 2010 and that I exerted all efforts to locate the same but I could not remember exactly where I placed it and it is considered lost for all purposes;

As such, I am executing this Affidavit of Loss to attest to the truth of the foregoing and to support my application for the issuance of a new Automated Teller Machine.

I, Michelle Sanchez, of legal of age, single and presently residing at 123 Hillside Street, after having duly sworn to in accordance with law, hereby depose and say that:

That as a legal student of ABC School, I was issued a Student Identification Card (ID) No. 11-111-111;

That I used to put the said Identification Card in my bag, among others;

That to my surprise, I discovered last January 5, 2010 that the Identification Card could not be found;

That I might have unintentionally dropped the said Identification card when I was going home from school;

That diligent effort to locate the Identification Card was useless;

That I am executing this affidavit to establish its loss and to support my request for replacement of the said Identification Card from ABC School.
